1. What's your favorite restaurant and why?
Hmm. Do I have a favorite restaurant? I'm a big fan of Ruth's Chris Steakhouse ... not that I can afford to go there, like, ever. :-)
2. What fast food restaurant are you partial to?
Wendy's.
3. What are your standards and rules for tipping?
Fifteen percent for the absolute bare minimum of acceptable service. If the service is bad enough that the server deserves less of a tip, I speak to management. I normally aim higher and try to tip around 20%, with the result that I probably average around 18%. I seem to tip on the whole amount of the check including tax, so I suppose I do pretty well by the waitstaff. Comes from being an ex-waitress myself.
Furthermore, there are two things I try to impress upon all my friends about eating out: (a) always overtip at breakfast, because the food is cheaper so you can't make as much money no matter how hard you work; and (b) tip in cash whenever possible, because that way there's less of a paper trail and the server doesn't get taxed as hard. (The third rule is, if the food takes forever to get to you but it's hot when it arrives, blame the kitchen and not the server -- but that's not a rule about tipping. [g])
4. Do you usually order an appetizer and/or dessert?
Almost never.

Diet Coke.
